The ideal candidate will provide high-quality, patient-centered care in an outpatient setting, focusing on the prevention, diagnosis, and treatment of a variety of medical conditions.

This role involves collaborating with physicians, nurses, and other healthcare professionals to deliver comprehensive healthcare services to our patients.

Key Responsibilities:

• Conduct comprehensive patient assessments, including obtaining medical history and performing physical examinations.
• Diagnose and manage common acute and chronic health conditions in accordance with clinical guidelines.
• Develop, implement, and monitor individualized care plans for patients, ensuring evidence-based practice and adherence to clinical protocols.
• Order and interpret diagnostic tests such as laboratory work and imaging studies, and make referrals to specialists as necessary.
• Educate patients and families on health promotion, disease prevention, and management of chronic conditions.
• Collaborate with other healthcare providers to coordinate patient care and improve health outcomes.
• Document patient encounters, clinical findings, and care plans accurately in the electronic medical record (EMR) system.

Qualifications:

• Education: Master’s or Doctorate degree in Nursing from an accredited Nurse Practitioner program or Physician Assistant - Certified.
• Must have current and valid state licensure as a Nurse Practitioner or Physician Assistant.

Skills:

• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Proficiency in electronic medical records (EMR) systems.
