Provides psychosocial services and support to patients and families through all phases of transplant journey. Conducts psychosocial assessments to identify patient strengths and risks related to individual’s suitability for transplant and develop appropriate treatment plans to overcome identified barriers and mitigate risk. Follows institutional guidelines for compliance and adheres to regulatory accreditation board policies. Collaborates with multidisciplinary team, patient, and family to develop and implement social work interventions. Effectively communicates and documents psychosocial recommendations, progress updates, and treatment plans to all members of health care team. Provides psychoeducation to staff, patients, and families. Maintains positive working relationships with vendors and external agencies to access services for patients. The individual must possess the ability to utilize critical thinking skills, decisive judgment, evidence-based practice, and professionalism.
Provides social work services for facilitating solutions to the medically related psychosocial/environmental problems of patients and families. Ensures the continuity of patient care by implementing interventions which may include initial assessment, identifying mental health disorders, brief behavioral health interventions, information and referral, during hospitalization and/or outpatient treatment. May provide educational support to staff. Maintains positive working relationships with vendors and external agencies to access services for patients. The individual must possess the ability to clinically assess and treat emotional mental health issues. The individual will also be able to diagnose under the direct supervision of a LCSW social worker. This LMSW position will assess data reflective of the patient's status and appropriately interpret information relative to the patient's age-specific needs is required.
QualificationsMaster's degree in Social Work. One year of current experience in a clinic, hospital, medical setting, or healthcare field preferred.Must have knowledge of adjustment to medical diagnosis and chronic disease. Strong communication skills are needed to interact with patients, family members/ care givers, providers and allied health staff. Must have psychosocial diagnostic skills and ability to coordinate transition of care. Active license through Arizona State Board of Behavioral Health Service Examiners; Licensed Master Social Worker (LMSW) required.
Exemption Status Exempt Compensation Detail $66,435 - $99,736 / year; Education, experience and tenure may be considered along with internal equity when job offers are extended.
